Keiko 'knew' Miles was alive because she saw him drink coffee in the afternoon on the security log, which he 'never' does, thus proving to her that the record of his and Bashir's deaths was forged.
However, it turns out that Keiko didn't know ALL of the Chief's mannerisms, since he says he "always" drinks coffee in the afternoon, so the whole thing was a glorious accident!
btw, I think the episode is called "Armageddon Game"

About Troi, I remember a quote by Odo from that ep. Troi starts being conversational, and Odo is observing the power conduits (which prevent him from shapeshifting his way out). Remember Akorem Laan? (or some such spelling) He entered several hundred years before the series! It is just that since he didn't come back out until DS9, nobody knew about it!

True..... forgot about that.
Then it was Dax and Sisko, so the Trill and Humans were the first to see where it went.
The first Gamma Quadrant species through the wormhole (officially) were the Wadi (Move Along Home, DS9), but I think you're right, the Tosk and the Hunters were probably the first, but not officially.
